This page services extension requests for:

• Accounts that do not have access to an online password
• New accounts

If you have an account and an online filing password, please apply For the extension through the Taxpayer’s Online Account. Otherwise provide as many account identifiers as available.

You can search for an account number through the Acct Search feature. Once the account number is entered, click the lookup icon to auto-fill the remaining account information. Fields marked with ** are required, unless otherwise specified. Upon submission, a copy of the request will be emailed to the specified "Email of Requester". Please attach a copy of the email to your return.

The Reset Form Option will clear the account information, while leaving the requester-related information, to streamline additional extension requests.
